Parent Information & Forms
Participation in the Trinity Athletic Program is a privilege, not a right. Upon making the decision to participate, student-athletes and parents must be prepared to make commitments.
In addition, participation requires student-athletes and their parents to read, understand and agree to abide by the policies and procedures detailed in this handbook. Additionally, student-athletes agree to abide by the Trinity High School Code of Conduct which governs all students, as well as all policies and bylaws contained in the handbook of the Kentucky High School Athletic Association (KHSAA). The information in the Parent Handbook is very important. Please take the time to read it carefully!
All student-athletes must complete the KHSAA Athletic Participation/Physical Examination/Consent and Release form and turn it in to the Athletic Office prior to tryouts or the start of the season. All other release forms need to be completed online. Please log in to your Power School account, click on forms and complete all items under Athletics/Activities. These forms are mandatory to participate in athletics. KHSAA rules and insurance requirements dictate that student-athletes and parents complete all forms. Understand that there are no exceptions to this policy.
